I agree with you, Elana.

Solutions are required:
- Tools to collect, track and evaluate known issues. Every automatic
tool starts with a human understanding the problem and automating the
previous (at least once) human-thought-through steps.
- People to work together requires a dataset, e.g., a list of known
issues, to work together on.
- Corrective actions require a list of known issues to begin with.

It all starts with pulling together a list of known issues from
already published sources.
